WebFor 1 specific district, none of the local deathcare facilities will pick up the dead. According to the traffic routes, anytime someone dies in 1 specific district, the hearse comes from off the map entirely (not even from a different district) causing them to take too long to get to the destination in many cases. Hit the info icon left of centre on the main hotbar bottom of screen. It's possible your first settlers got old and are dying off.
